Laproscopic cholecystectomy
Definition : Surgery to remove the gallbladder is called a cholecystectomy.
The gallbladder is a small pear-shaped organ that stores and concentrates bile.
Patient Name: Moses Pawar
Date of Surgery: 31/10/2006
Patient Admission Time: 31/10/06 9.30 am
Patient Discharge Time: 31/10/06 2.30 pm
Performed by: Dr JUDITH KEDDINGTON
Reason for surgery : Gall bladder stones.
Patient accompanied by : Wife Preeti kadam ( herself a registered nurse )
Patient was admitted at 9.30 am and immediately asked to undress and put on a surgery gown. About 4 nurses repeated a same set of questions:
a) history of hypertension / TB / ? - No
b) any current exposure to Hepatatis/HIV/ Cancer? -No
c) allergy to any medicine/anesthesia? - Not that patient knows of
d) Smoker/ been a smoker? - No
e) Alcohol/recreational drug usage? - No
After that a nurse tried to insert an intracath in a vein located on patients dorsal side of the right palm. Attempt was unsuccessful due to a blocking valve in the vein. Attempt did cause patient significant pain. Patients wife criticised (ofcourse quietly to the patient in a language not understood by the nurse ) for not doing the procedure correctly.
Another nurse tried the same procedure this time on the vein located on medial side of left wrist joint with exact above result ( including criticsim from Patients wife )
On suggestion from nurse anesthetist the procedure was tried on a vein in the left antecubital region. This time with successful outcome.
Nurse anesthetist also went over possible risks of general anesthesia to which the patient nodded and agreed. What was he going to do anyway , incase the risks became true? She also conveyed that Endotracheal Intubation would be done to avoid gastric fluids from inundating the lungs and causing pneumonia.
After passing IV fluids into the patients body he was taken into the surgery room. General anestesia was given. Laproscopic cholecystectomy was performed.
After surgery patient's heart rate slowed down considerably to around 35-36 beats/minute. On duty doctor was informed. Observing patients blood pressure , conclusion was this was not a serious condition and patient be discharged after half an hour of monitoring.
Patient was wheel chaired out by a volunteer to the main lobby where his wife was waiting for him . She drove him home.
Patient felt some pain but over all condition looks good. Patient thanked God and all those who prayed for him and looks forward to speedy recovery.
